Guidelines for Entry -

The fee for each entry is $99. To submit multiple entries, select the number of entries you wish to submit in the form below, choose the number of entries to submit before you checkout.

Each finalist will receive a video package presentation and the ability to accept their award live on stage at our NEW YOU Awards event. See and example below:
